M.G. Lewis Scholarship Application

Grants for one academic year are not to exceed $1,000.00.

Please complete each of the four easy steps below through our secure on-line site.  You will receive a confirmation email after completing each part. It is your responsibility to ensure your application is 100% complete. Incomplete applications will not be considered.

Step 4. M.G. Lewis Scholarship Application Part IV - Please note that this is a three-part step:
  1. Include Personal Statement of Need & Merit (700 words or less)
  2. Upload Letter of Recommendation, and
  3. Upload Transcripts. 

As an alternative, Letters of recommendation & official transcripts may be emailed to scholarship@southsidemethodist.org OR mailed to the church office at 3120 Hendricks Avenue, Jacksonville, FL 32207 attn: Scholarship Committee.
